Your first visit

Your initial dental visit should be scheduled at a time that is convenient for you to spend an hour or more in the office. The initial visit is generally scheduled to include a comprehensive examination by the doctor and any necessary radiographs (x-rays). Whenever possible, a prophylaxis (dental cleaning) will also be performed at this visit. A comprehensive treatment plan for recommended dental procedures will be provided to you at the completion of your visit. The doctor will be available to answer questions about treatment, and our treatment coordinators will be available to answer questions about costs, insurance and financing options. A scheduling coordinator is always available to assist you in scheduling a follow-up visit.

We require each patient to complete several forms prior to being seen for an initial visit. For your convenience, we have included these forms on this website. If you choose, you may download these forms and complete them prior to coming to the office for your initial visit. Link to forms.
